To obtain additional visa pages, please submit the following documents:

1. Original valid US passport. (in a good condition)
2. Application form DS-4085 filled out and signed.
3. Authorization letter filled out and signed.|
4. Flight Itinerary.
5. Payment (cash, money order, check payable to CTVC or *credit card + 5 %)

Processing time does not include the day of submission to CTVC and mailing time.

6. SELF-ADDRESSED PREPAID ENVELOPE or extra $ 30 for FedEx overnight

In case of denial by Passport Agency or cancellation or amendments by client, processing fees is not refundable.
Processing time does not include mailing time.
