2. Passport amendment form DS-19, GREEN

3 . Proof of travel, a copy of your itinerary

4. For a NAME CHANGE submit ORIGINAL documents only:

5. A FULLY completed and signed copy of the CLIENT AUTHORIZATION LETTER

• Original Marriage license
• Original Divorce Decree
• Original Court order showing the name change

6. Payment made by check, money order or credit card made to CTVC

$235.00 for 72 hours processing.

For 24 hours processing please add $100.00.

7. Self-addressed stamped envelope. Express mail is recommended, if processed documents are to be returned by mail or please enclose $18.00 for priority overnight express mail (up to three passports).
